You may apply for a duplicate license if you are changing your name or address; do not have your license in your possession (lost/stolen) or your license is damaged. You must appear in person and present your driver license, or, if you have lost or damaged your license, you must provide (2) identifiable documents (i.e. social security card and birth certificate).
- Complete and print the Driver License Application for duplicate license (please bring with you);
- For name changes, present acceptable documents for name change (certified marriage certificate, certified divorce decree, certified name change document)
- For address changes present two (2) proof of principal residence address. If your post office box is your mailing address, you must also provide a document with your physical address listed.
The fee for a duplicate license or identification card is $6. You must apply in person to make a material change on your driver license or identification card.
Address Change By Mail
To change your driver license or state identification address by mail, please fill out the Change of Address Form then print, sign and mail the form to the below address.
Mail in two (2) acceptable documents addressed to you at your new address (If your post office box is your mailing address, you must also provide a document with your physical address listed). The address change will be on your record only.

Your original license expiration date will remain the same. If you are eligible for a license renewal (six months prior to expiration) we recommend you apply for a license renewal instead of a duplicate license. See the “Acceptable Document List” for a listing of all acceptable documents.
