For applicants who are under the age of fourteen years, the application shall be made on the minor’s behalf by the parent, or by another individual in loco parentis of the minor who can provide proof of guardianship.  In the case of an incapacitated/incompetent individual, the application shall be made by the individual having custody or control of or maintaining the incapacitated/incompetent individual as indicated in appropriate documentation.

• Affidavit for Incapacitated/Incompetent Individual

All initial and subsequent renewals must be made in person. If an applicant is eighty years of age or older, the renewal may be accomplished by mail, provided that the applicant had been processed as an in person initial issuance or renewal after January 1, 2013. Applicants must provide documentary proof of legal name, date of birth, social security number, legal presence and principal residence address.

• U.S. State, Local Government Certified Birth Certificate .
• Valid and unexpired U.S. Passport.
• Military Documents: ID Card and US Military issued Certificate of Birth. ID Card must contain photo, Date of Birth (DOB) and name. DD-214 with name, DOB and social security number must match other submitted verification documents. DOD Common Access Card/U.S. Uniform Services ID and Privileges Card must contain photo, name and DOB
• Consular Report of Birth Abroad.
• Valid and unexpired Employment Authorization Document.
• Unexpired foreign passport with a valid unexpired U.S. visa affixed accompanied by the approved I-94 form.
• U.S. Certificate of Naturalization
• U.S. Certificate of Citizenship
• Valid Permanent Resident Card issued since 1997
• Valid and unexpired Temporary Resident Identification Card
• Valid and unexpired TSA Transportation Worker Identification Credential

Effective June 18, 2021, presenting documentation for social security number is optional.  One of the following documents may be presented but is no longer required.  Documents may be required if we are unable to verify your number.

• Original Social Security Card (no copies or metal plates). Need a Social Security card?
• Military Documents: ID Card or DD-214, provided the Social Security Number is printed on the document. Original or certified copies of DD-214.
• Wage and Tax Statement Form W-2 or 1099R, provided the Social Security Number is printed on the document. Original or certified copy. Printed copy of electronically transmitted form acceptable.
• SSA-1099 form, from a previous year. Original or certified copy.
• Non SSA-1099 form, from a previous year. Original or certified copy.
• A pay stub with the applicant’s name and Social Security Number printed. Original or certified copy.

Note: Additional documents may be required if there is a difference in name on the documents submitted. See Acceptable Documents for Change of Name. Laminated cards are not acceptable.

• U.S. State, Local Government Certified Birth Certificate.
• Valid and unexpired U.S. Passport.
• Consular Report of Birth Abroad.
• Valid and unexpired Employment Authorization Document.
• Valid and unexpired foreign passport with a valid unexpired U.S. visa affixed accompanied by the approved I-94 form.  Appropriate immigration document(s) determine legal presence, not the passport itself.

Two (2) documents required (if your post office box is your mailing address, you must also bring in a document with your physical address listed).

Note: Present any two of the following documents that contain the applicant’s name and the address of the applicant’s principal residence within the State of Hawai`i. If applicant is not the addressee, an affidavit must be made by the addressee that the applicant’s principal residence is the same as the addressee. • Vehicle registration or Title issued at least thirty (30) days before the application date
• Valid Driver’s License
• Voter Notification Postcard (yellow card) or other mail addressed to applicant from a government or medical entity not more than two (2) months old
• Utility bill with the applicant’s name and address (not more than two (2) months from application date)
• Checking or savings account statement not more than two (2) months old
• Payroll check or check stub with applicant’s name and address issued by an employer within two (2) months of the application date
• Mortgage account or proof of home ownership
• Residential Rental or Time-Share contract for six (6) months or more (Month-to-month agreements will not be accepted)
• U.S. individual income tax return, W-2 form or income tax form 1099 form from the previous year
• Hawai'i individual income tax return or HW-2 form filed in the last two-years
• Letter from the Hawai'i State Department of Human Services indicating the person is receiving welfare assistance dated not more than 90-days old.
• Receipt for personal property taxes paid
• Medical card issued by a health insurance agency, provided the principal residence address is printed on the card
• Documentation or valid identification card that the applicant is receiving State of Hawai'i public assistance (health benefits, social services program, etc.) dated not more than ninety (90) days
• Current Property Tax Assessment Form
• A stamped Department of Taxation Form A-6 (Tax Clearance Application)
• Homeless applicants may use the address of their current shelter agency or, if not staying in a shelter, may use the general delivery of the post office nearest where they spend most of their time
• Applicants documenting enrollment in a State or Federal address confidentiality program which allows an applicant to obtain and use alternative addresses may use an alternative address on the card, but must provide the applicant’s permanent address for file purposes.
• The County of Hawai'i will accept a document addressed to you at a P.O. Box address.  You must present a second document with your physical address. 
• Hawai'i principal residence address affidavit, provided the spouse, parent's, guardian's or other's address can be verified and the affidavit is notarized within two-months of the application date.  Contact your county DMV office for further information.
• Homeless applicants may use the address of their current shelter agency, or if not staying in a shelter, may use the general delivery of the post office nearest where they spend most of their time.  In addition, for state ID applicants, a signed sworn statement from a victim service organization, an attorney, a member of the clergy, correctional institutional staff, a medical or other health professional from whom the person has sought services, or a verification letter from a homeless service provider as documentary evidence of the person's address.
